The Canadian Dental Care Plan covers many dental care services, such as:

Eligibility Criteria

To qualify for the CDCP, you must:

  • be a Canadian resident;
  • have an annual adjusted family net income under $90,000;
  • not have access to private dental insurance; and
  • have filed a tax return for the previous year
How to Apply for the Canadian Dental Care Plan (CDCP)

Here’s how you can submit your application.

  1. Confirm that you meet the eligibility criteria.
  2. Apply by mail or online by visiting the official Government of Canada website (See button below)
  3. After applying for the Canadian Dental Care Plan, Health Canada & Sun Life will review your application which may take from 4 to 6 weeks.
  4. When approved, you will receive a confirmation that includes your coverage start date, membership information, and other relevant details.
  5. Book your appointment and let our front desk know you are covered under the CDCP.

What do I need to apply?

After verifying that you eligible for the program, please gather the following information for each applicant

  • Social Insurance Number (SIN) (if available for children)
  • Date of Birth
  • Full Name
  • Home and Mailing Address
  • List of Dental coverage through government social programs (if applicable)

The CDCP covers a percentage of the Government of Canada’s established Fee Guide, which may be different than our clinic’s rates.

Our team will inform you if there’s any remaining balance before the treatment begins.

The CDCP covers the following services without pre-authorization:

  • Examinations, Radiographs, Lab Tests
  • Prevention Restoration
  • Endodontics, Periodontics, Prosthodontics
  • Oral and Maxillofacial Surgery
  • Adjunctive Services

The following services are covered with pre-authorization:

  • Diagnostic, prevention, restoration, Endotonics, Periodontics, Prosthodontics, Oral and Maxillofacial Surgery, Orthodontics, adjunctive services
